Empire United Coach Reimbursement Form

Dear EUSA Buffalo Staff,

Please fill out the form below as soon as you return from your trip.  Fill it out completely and then mail your receipts to me at the address below.  A copy of this information will go to our treasurer and to me.  Your online team budget will be updated shortly thereafter.  Remember, the goal here is to make sure all of your expenses are covered while at the same time spending as little of the team's competition budget as we can.

Approved Trips for Reimbursement

  • Any team tournament
  • State Cup and Regional Weekend Finals
  • Region 1 League games (Colonial or Premier League)
  • Great Lakes Alliance Friendlies (GLA's)
  • Thruway League double headers requiring an overnight stay (pre-approval required)
  • Thruway League games scheduled more than 2 hours from your home (only car and meal per diems, gas, and tolls apply)

Notes

Assistant coaches will get meal per diems where applicable, but will not be reimbursed for other travel expenses unless pre-approval is made.  Assistant coaches are expected to travel and stay with head coaches to reduce team travel expenditures.
